Operational systems become difficult to trust when they show what a team decided but not how the decision was reached. Auditability begins by modelling the case as a sequence of attributable events rather than a row that is repeatedly overwritten.
Separate the case from its evidence
The case represents the operational question and its lifecycle. Evidence items retain their own source, time, identity, files and version. Linking them allows new evidence to be added without erasing the earlier basis for review.
Record the actors and transitions
Useful case histories identify who created, viewed, assigned, changed, reviewed or closed a record. Transitions should capture the previous state, new state, time and reason—not merely the latest label.
Core structures
- Case identity, type, priority and lifecycle state
- Evidence items, sources, files and hashes
- Assignments, roles, teams and access history
- Tasks, actions, correspondence and deadlines
- Review rationale, uncertainty and decision boundary
- Exports and reports linked to the case version that produced them
Design permissions around responsibility
Not every participant needs the same view or authority. Role-aware access should reflect who can gather evidence, perform review, approve action, manage users or inspect the audit history. Sensitive fields and exports may require tighter controls than general case data.
Make exports part of the model
Reports and data extracts should retain stable identifiers and explain which records and versions they contain. This makes it possible to reconcile an exported evidence pack with the operating system later.
